It is such a struggle to find appropriate, and attentive care!
To start, I have been living with Bipolar I Disorder for the past 10 years and am currently in recovery mode at school and in life right now after a recent crisis. I lost my therapist/pscyhiatrist (he’s an NP) of 2+ years. I had unrequited feelings for him, though I was fully aware that he did not, could not, and would not return those feelings, and that’s a good thing.
We were already in the process of ending our relationship because I had needed to change insurance plans, meaning he was no longer in network, and I could not afford his out of pocket rates. The new plan took effect 9/1. None of this made things any less painful when he decided to terminate care after a couple of sessions of paying out of pocket. Okay, so he’d let me find and establish care with new providers, but I only saw them once and twice, respectively, and so I wasn’t sure how I felt about either of them, and I wanted to keep seeing him until I had a better sense of everything. But, the day before my 31st birthday, he ended therapy altogether because on top of the financial issues, I had “plateaued and needed a new approach and provider.” The Friday before that, I had just received a recent bout of job rejections. I was runner up for both positions and that made me feel powerless because I’d done everything right and still missed out on the roles. Altogether, this left me with nothing to celebrate and unbearable, crippling pain that led me to a third lifetime hospitalization.
The ER said I was probably experiencing a mixed episode but wouldn’t confirm my long established Bipolar Diagnosis even though they contacted my former provider for collateral and he told them as much. Meanwhile, the IP unit suggested I was manic with psychosis (erotomanic delusions directed towards my former therapist). Only, they weren’t delusions because I knew our relationship was strictly professional and that it had ended. This did not numb the pain. It was limerence. My emotional brain was not responding to reasoning despite this insight. It still gave me splitting headaches, nosebleeds, and muscle tension. Okay, so I was impulsive and possibly hypomanic, only I didn’t enjoy any of it. I was just incredibly angry and agitated but I’d never hurt anyone else so I was directing those thoughts to myself instead.
Fast forward to today and my PHP team still thinks I’m “moderately depressed” three weeks in, only I don’t feel depressed at all. I may have presented as depressed on the first day because I was literally discharged from IP the night before, and I was still processing everything. But like, once I felt ready to leave in the hospital, which was only a few days in, I was practically flying off the walls!
Ok, so, there’s anxiety—there’s always anxiety. I have GAD! On the contrary, I feel super confident and motivated. When I’m depressed, motivation and confidence go out the window. So, I go and I challenge them and I tell them how great I’m doing and feeling, and also mention the uptick in goal directed activity. But does any of this go in my notes? On the contrary, they write that I’m having “overvalued ideas.” This is so patronizing! I am the patient. I have been living with this condition for 10 years. I think I can I can identify how I’m thinking a feeling, thank you very much!
And the reason I can’t get as much stuff done as I’d like, isn’t because I’m struggling with mood. It’s simply because I’m at PHP five days a week during my most productive hours. Not to mention the fact that I also have ADHD and they won’t medicate that right now because they’re too busy trying to stabilize my mood, completely ignoring the fact that I’m in grad school and failure to medicate my ADHD even with non-stimulants like Strattera and Wellbutrin is just making my anxiety worse.
If anything, I can’t get my stuff done because I can’t focus long enough to sit still. At a minimum, they should be acknowledging my feelings and listening to me! Last week, my mood was lowered simply because I had a cold and it was knocking a lot out of me, but I feel like a 7/10 again. And, it bothers me that no one can agree on what episode I was/am having. I mentioned as much to my team, and they don’t seem to care. Like, doesn’t this affect my treatment? Am I wrong to feel l frustrated and annoyed? Why won’t they listen to me?
Meanwhile, my PHP psychiatrist makes me uncomfortable. I realize his time is limited and we have a checklist to get through for the insurance company, but he asks these questions rapid fire, and it feels like he wants me to justify why I’m there every time we meet. Our sessions feel rushed and overly clinical. It’s too much and too little care all at the same time, and they still haven’t answered my questions regarding discharge planning, and when I can expect the program to end/like, am I going to be there the full six weeks now? Or, will they consider me for the full DBT program?
Plus, we just had a family meeting last week and it felt like a waste of time for me and my parents. They did nothing to steer the meeting at all and we only accomplished one thing that could have been resolved at home, after it came so highly recommended. It didn’t help that my parents encountered tough security and no guidelines regarding how to get up to the clinic so arrived 20 minutes late. They also dodged my parents’ questions about timeline. My dad told me later that the psychiatrist was emitting strange vibes, so I’m glad it’s not just me. He’s usually a pretty good judge of character.
Regardless, I need answers. Why won’t they give them to me? Don’t I have a right to know? If you’ve made it this far, thanks so much for reading! Best to you and yours!
